There are several ways to submit your form:

1. Print out the form and hand write in your information and submit it via fax or regular mail to the Panhandle AHEC.

2. Fill the form in using Word, save it to your computer and ATTACH it to an email with "School Registration" in the subject line.

3. Fill the form in using Word, print it and FAX it to the Panhandle AHEC
